MEDICAL FORM AB DOWNLOAD

ALL participants will need to fill out the 2023 Camp Long Lake “Pre-Event Medical Screening Checklist”.

This is a tool to assist leaders in identifying potentially communicable diseases in advance of event participation. The intent of this checklist is to review with each participant their current health status both before departure and upon arrival at the event.

NOTE: These will be available at camp if you are unable to download and fill out prior to the event.

This form is to be filled out ON FRIDAY May 12th as it pertains to how you are feeling at that time.

GEAR: What should we bring??

What each family brings is entirely up to them.

Note that it is a short walk to our camp site, and you will not be able to drive up and drop off your gear.

Below you will find a ‘Family Camping Checklist’ that is provided by the Potawatomi Area Council.

Here are some items we recommend:

1. WAGON

This is HIGHLY recommended to get your gear to and from the camp site.

2. RAIN GEAR

The weather has historically been wet.

3. RAIN BOOTS/WATERPROOF SHOES

The grass will have dew on it in the morning, which will lead to walking around in wet shoes all day. Wet shoes = Stinky feet.

4. EXTRA SHOES AND SOCKS

Always a good idea to have a spare, or two.

5. COLD WEATHER GEAR

The weather will most-likely be cold and wet, but might be warm and dry during the day. Plan accordingly.

6. SMALL COOLER/SNACK

A small cooler with snacks and a few drinks for each day is recommended. This is just incase you or your kids get hungry in between each meal. The meals are filling so you will not need much. Also note, if you have special dietary needs, a cooler is HIGHLY recommended.
